Can nothing be done to relieve this situation?' The above was reported to the Perth Hospital Board by the Principal Almoner in 1945. In 1963, the Western Australian Public Health Department, realising that there was an important medical aspect to this dilemma, decided it was time to introduce a geriatric service to Western Australia. Geriatric Medicine is a fascinating chronicle of the early days of this service, from its small beginning in one of the State's two so-called 'benevolent homes' - these, the relics of the asylum era when custodial care was the accepted solution to the health and social problems of old age. Dr Lefroy's sensitively written account describes how rehabilitation, provision of home care, and attention to people needing residential care became the basic principles of a geriatric service in WA. Assessment of social and medical needs of elderly people was deemed paramount in deciding the most appropriate kind and level of care. With WA's pioneering approach to a geriatric service, people in country centres at last had a choice of remaining at home, rather than being transferred to Perth. Hostels, including ones catering for people with mild dementia, were another aspect of aged care that was to receive attention in the State. In the concluding chapters of Geriatric Medicine, Dr Lefroy reflects on some of the special aspects of attention now given to elderly people with disabilities and chronic illness. He also describes how geriatric medicine became a recognised area of study for medical undergraduates and a specialty within medicine, as well as gaining an accepted role in acute hospitals today.
